  2. Model II additionally includes: family history of diabetes mellitus (yes or no), family history of cardiovascular disease (yes or no), body mass index at age 50 years, education level (9 years or less, 10–12 years, > 12 years), smoking status at age 50 years (yes or no), and year of birth
  3. Model III additionally includes: capillary glucose at 2 h following 75 g OGTT at age 50 years (for T2D) or mean arterial pressure at age 50 years (for hypertension)
