Active smoking was regular tobacco use at or within 12 months prior to enrolment. Regular alcohol use was consumption of any alcoholic beverage >3 times/week. The HOMA (Homeostasis Model Assessment) index is the fasting glucose concentration (in milligrams per deciliter) multiplied by the fasting insulin concentration (in microunits per milliliter) divided by 405.
